Job Description

Employment Term: Part-Time (28hrs per week)

Location: Auchenflower

This position will support our weekend staff and act as an on-call for shifts during the week, must have the ability to work weekends.

We are currently seeking to appoint a Case Worker to work as a part of our Clare Homes team. Our Case Workers support families experiencing or at risk of homelessness with housing case management whilst they are in short-term accommodation. This position involves providing mobile support in the community.

As part of this role, you will:

  • Deliver rapid and coordinated assistance to people in critical need of housing and/or support through triage, assessment, case planning and crisis intervention.
  • Respond and action referrals identified during case management with families.
  • Adhere to program guidelines regarding the appropriate use of allocated funds.
  • Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate records within the appropriate timeframes.
  • Identify and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff.
  • Positively represent the organisation when engaging with all stakeholders.
  • Undertake other duties as and when required within the scope and capabilities of the role.
