Job Description

The Going Places Program aims to assist clients who are rough sleeping or experiencing chronic homelessness transition into long-term stable housing through providing outreach support and strengths based case management, utilising a ‘housing first’ approach. The role is to address the complex health needs of clients of the Going Places Homeless Program by providing outreach support, comprehensive health assessments and support to access the appropriate health services.

We currently have a vacancy for a Clinical Nurse to join the team on a full time permanent basis.

Your key responsibilities will be to:

Deliver a range of clinical and psychosocial interventions with clients
Provide support, direction and education to Registered Nurses and lead the health team
Conduct comprehensive health assessments, including risk assessments
Conduct patrols of the city area, including known homeless meeting places
Actively participate and engage in case conferencing, interagency meetings
Complete a range of internal and external reports
