HART 4000 Homelessness Intervention Worker

Work a 9 day fortnight and make social impact, connect with individuals and build a stronger community around homelessness.
We have an exciting opportunity to recruit to our vibrant team at Communify's HART 4000.
The Homelessness Intervention Worker will be part of a dynamic team based at HART 4000 office in Fortitude Valley. This 9 day fortnight part time position aims to connect, refer and communicate with relevant support agencies. It will involve in-house support and active referrals to assist people to link with the services they need.

We are looking for someone with:

Social Work degree and/or extensive experience in human services
Demonstrated skills and knowledge of community based intake, assessment, case management & documentation procedures
Demonstrated excellence in interpersonal, written and organisational skills
Ability to establish & develop links with communities, communicate & liaise with service providers & work with complex needs clients to provide good case management
Current valid drivers licence
Current Police Check and Blue Card or the ability to obtain them
