Under the guidance of the Support Coordinator, the Housing Support Worker is responsible for delivering case management services and assisting households in crisis to secure and maintain appropriate housing.
Within this position you will:
Conduct risk assessments and implement strategies to ensure client safety and well-being;
Develop, implement, and review strengths-based, needs-driven case management plans;
Assist clients with budgeting, tenancy management, stakeholder communication, and accessing specialist support;
Record client interactions and program participation, and complete monthly and annual compliance reports;
Network and partner with stakeholders to provide comprehensive, coordinated care and holistic support.
This position also entails home visits and providing mobile support to families within crisis and emergency accommodation. Location is based across Brisbane, Moreton Bay, Logan and Ipswich regions.
