AOD Outreach Social Support Worker
Full time position available
servicing either Brisbane South and Brisbane North
In this role you will work with clients in a case management capacity, including travelling to identified outreach areas:
Individual Engagement
- Provide an outreach case management program
- Ensure clients accessing the service are treated in a professional and respectful manner always, and in accordance with the Client Services Charter of QuIHN
- Work collaboratively, with other services to provide AOD support to the homeless community
- Provide brief interventions which may include one off information, psycho-education and crisis support
- Provide health care information, harm reduction strategies, education for people experiencing problematic substance use, and the wider community
- Support clients with welfare needs, housing, education and employment, through active referral pathways as part of a case management plan
- Maintain effective referral pathways that will meet the needs of a complex client group
- Participate in case conferences, in a case management capacity supporting the development and implementation of case management plans and safety plans
- Advocate on behalf of clients with other service providers
Community Engagement
- Maintain effective referral pathways that will meet the needs of a complex client group
- Participate in inter-agency and multi-disciplinary networks relevant to the range of services
- Provide information sessions and general liaison with other agencies to market QuIHN programs, with a particular focus on facilitating referrals into and out of these services
