Job Description
The Supportive Housing – Hope Street team are based at Brisbane Common Ground. Each element of Supportive Housing, from the building to support service design, aims to create the greatest degree of empowerment and independence for people. Supportive Housing – Hope Street uses the Housing First model.
As a Support and Advocacy Worker you will work as part of a multidisciplinary team to advocate, provide holistic case management services for the tenants of Brisbane Common Ground and to empower them to make choices that improve their quality of life. Also to help tenants foster independence and assist them to access appropriate resources, opportunities and services within the community.
Key Responsibilities
- Provide case management support to tenants as part of a multidisciplinary team.
- Engage with tenants in various settings, including Concierge, common areas, and their units.
- Deliver excellent customer service and problem-solving in high-pressure environments.
- Liaise professionally with all stakeholders.
- Promote a culture of respect, safety, and trauma-informed approaches.
- Assist with administrative, practice, and quality requirements.
- Maintain accurate and complete data and case notes.
- Participate in supervision and professional development activities.
- Manage self-care and contribute to a safe workplace.
Essential Criteria
- Commitment to child and vulnerable adult safeguarding principles.
- Proficiency in IT systems, including case management and data collection.
- Positive attitude and ability to work flexibly and proactively in a team and autonomously.
