Job Description

As a Support and Advocacy Worker within the Home for Good team, you will provide centre-based or outreach services to individuals and families experiencing homelessness or at risk of homelessness. You will work collaboratively to deliver support, advocacy, and health care plans, ensuring the immediate needs of participants are met in a supportive, informed, safe, and respectful manner.

Key Responsibilities:

  • Promote a culture of respect, safety, and trauma-informed approaches.
  • Provide support through assessment, triage, and prioritisation based on participants’ needs.
  • Develop and implement support and health care plans.
  • Advocate and build relationships with internal and external stakeholders.
  • Prepare documentation for housing and service eligibility.
  • Submit referrals to internal and external agencies.
  • Transport individuals to support services or safe accommodation as needed.
  • Maintain accurate data and case management records.
  • Participate in supervision, professional development, and team meetings.
  • Ensure compliance with Work Health and Safety regulations.

Essential Criteria:

  • Formal qualification relevant to the role and/or demonstrated experience in a community-based environment.
  • Knowledge or capacity to acquire knowledge of solution-focused and problem-solving approaches.
  • Strong engagement and interpersonal skills, with a non-judgemental approach.
  • Effective verbal and written communication skills.
  • Experience working with diverse populations, including Aboriginal and Torres Strait Islander peoples.
  • IT proficiency, including data and case management systems, Outlook, and Microsoft Office.
  • Positive attitude, flexibility, and ability to work both autonomously and within a team.
