CHHII model of coordinated care has first panel meeting

After many months of designing and planning, the first CHHII panel convened this week. The CHHII Panel, comprised of 12 partner agencies (4 government, 8 non-government), considered a number of complex referrals using their respective expertise.  It was a wonderful opportunity for the community services sector to integrate their expert knowledge and resources to deliver quality coordinated care to people who are, or are at risk of homelessness in the Cairns region.

I would like to give a sincere acknowledgement to the CHHII Reference Group for their dedication in bringing the CHHII model of coordinated care to Cairns in the most challenging of times.  Members of the CHHII Reference Group include:
