Engaging Housing Providers in Care Coordination for improved outcomes for highly vulnerable individuals and families across Brisbane

The Service Integration Initiative: Brisbane Regional Care Coordinators  are  planning  a succinct forum for social housing providers to be able to develop a broader and renewed understanding of Care Coordination across the region.

The purpose, the processes, the benefits and how to move together past the challenges, as well as inviting providers to influence this transformational initiative, in committing to participating in one or more of the care coordination groups across Brisbane.
