It happened! An actual face to face Planning Workshop with the South West Care Coordination Group, Brisbane Region

A huge shout out to those of you who participated in Wednesday’s South West Care Coordination Group’s Planning Workshop – face to face and via Zoom. Thanks to Sherwood Neighbourhood Centre for hosting the workshop and keeping us fed, watered and socially distant!

What an incredibly rich discussion the 20 people representing 12 agencies all had yesterday regarding the Service Integration Initiative, the history and current status of the SWCCG, engaging in change processes and visioning Care Coordination within the South West Region. The dialogue and feedback generated from the workshop will inform the SWCCG Action Plan and how we move forward together as a region.
