Moreton Bay Care Coordination Planning Workshop
In Moreton Bay for October we are scheduling a Care Coordination Planning Workshop to develop the current Care Coordination groups and their supporting mechanisms to support more effective outcomes for clients and collaboration of services that align with the Service Integration Initiative.
This workshop will also inform services of the Moreton Bay Place Based Response Team, current Care Coordination Groups and Network Meetings.
Ongoing development and implementation of a Community of Practice for Moreton Bay workforce enhancement through consultation with the sector led by members of Place Based Response Team.
The Moreton Bay North & South Care Coordination Meetings are scheduled for October as is the Moreton Bay Housing & Homelessness Network Meeting – you can register to attend via The Deck.
