Redland Regional Care Coordinator
The Redland Region recently completed the Redland Care Coordination Forum and Regional Action Plan Forum facilitated by Stephen Hawkins of Qshelter. Thank-you to all those that participated, despite these discussions occurring via online conferencing we had a well rounded group of participants from diverse sphere’s of service provision and agency represenation. This included Department of Housing and Public Works, Redland City Council, Brisbane City Council, Child Safety, Youth and Women, Centrelink, Housing providers, Queensland Health, Specialist homelessness services, Mental Health and Addiction and Employment. Some robust conversations were shared in developing actions towards sustainment and enhancement of the Care Coordination meeting which will be further developed into an Action Plan to be shared and endorsed by participants of the Care Coordination Meeting. This was also the case with the Regional Action Plan Forum, the purpose of this forum was to share with the region the different mechanisms for service integration initiated by the Department of Housing and Public Works and develop strategies for connection, engagement and enhancement of these mechanisms. An Action Plan is also underway with the feedback provided by those that participated in the forum.
The Redland Care Coordination Forum occurs fortnightly with the next meeting occurring on the 13th October 2020 via Microsoft teams from 11.00am to 12.30pm and is facilitated by the Regional Care Coordinator Patrina Malopito.The Placed Based Response Team meeting which is the governance group for the Care Coordination meeting also occurs fortnightly and is facilitated by the Regional Director Mark McConnell of the Department of Housing and Public Works for Redland Region.
