The Redland Care Coordination Group would like to extend an invitation to the region to engage in a forum discussing the principles of Care Coordination. The meeting will also include a review of the current meeting and an opportunity to engage in action planning for the future of the meeting.
The Redland Care Coordination meeting was founded this year by the Bayside Housing Centre as part of a crisis response to COVID19. The group focuses on coordinated multi-disciplinary responses to complex cases of homelessness in the region. The initial group achieved some outstanding outcomes for homelessness for the region which included the Rapid Rehousing Response. With the engagement of the Regional Care Coordinator as part of the Service Integration Initaitive in May 2020 the group developed further in its governance frameworks, service delivery and inclusion of other specialist homelessness and support services for multi-disciplinary response.
The group continues to evolve and strengthen its practices and the forum will provide opportunity for this to continue. If you would like an invitation to engage in this forum and the care coordination meeting please click on the below link to register and an invitation will be sent to you.
