In the case you complete an After Care application on behalf of a young person we will send you a form to complete once goods and service have been obtained. There’s also a vehicle checklist and forms in the case a young person is seeking funds to support the purchase of a vehicle. Please let us know if you require this.

If you have any questions please feel free to contact myself or YHARS coordinator Naomi Donohoe at ndonohoe@wmq.org.au.
