This patient information booklet has been developed to assist people who are patients of a general practice or allied health professionals in the North Brisbane and Moreton Bay Region with key information in relation to accessing the National Disability Insurance Scheme (NDIS).
This booklet is for you, if you are a person with a disability or a carer of a person with disability who may be eligible for the NDIS.
