About the role
This position is 4 days per week, with Fridays preferred to meet service needs. Working within a youth homelessness service, you will support the delivery of high-quality, client-centred case management and intake services:
Your responsibilities will include:
- Managing a caseload of young people (aged 16–25) experiencing or at risk of homelessness.
- Conducting intake assessments, phone referrals and QHIP processes.
- Providing practical and emotional support, life skills development, advocacy and engagement.
- Supporting clients to develop and implement case plans, including coordinating referrals to health, housing, education and community supports.
- Assisting young people to maintain their NWYAS accommodation and supporting pathways to long‑term housing options.
- Liaising with government and community agencies, participating in case meetings and contributing to coordinated support responses.
- Recording case notes, maintaining accurate client data and preparing written reports as required.
- Contributing to team meetings, service planning, professional development and continuous improvement.
- Participating in client feedback processes and general operational support within the service.
About you:
- Qualification in Social Science/Human Services/Social Work.
- Experience in youth homelessness, community services or related fields.
- Strong case management skills, including assessment, planning and referral coordination.
- Experience with QHIP and/or homelessness intake processes (or willingness to learn).
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse young people.
- Strong organisational and time management abilities, with capacity to manage competing priorities.
- Sound understanding of trauma‑informed practice and the challenges impacting young people at risk of homelessness.
- A passion for empowering young people and delivering high-quality services.
- Works well within a small, friendly team.
