We’re looking for a compassionate and skilled Outreach Worker to join our dedicated team in our Maryborough Office supporting some of the most vulnerable people in our community.
About the Role
We are seeking a compassionate and motivated Outreach Worker to join our team supporting individuals experiencing or at risk of homelessness. This role involves delivering person-centred, trauma-informed support in community settings, including temporary accommodation and outreach locations in the Fraser Coast Region.
You will work with clients with complex needs, providing practical assistance, advocacy, and case management to help them achieve safe and sustainable housing outcomes.
Key Responsibilities:
Main tasks will include (but are not limited to):
- Provide outreach support to clients in a variety of settings (parks, sleeping in cars or on the street, motels, caravan parks and other community locations)
- Conduct risk assessments and develop safety plans in line with organisational procedures
- Deliver case management and develop individual support plans
- Support clients to access housing, health, mental health, AOD and community services
- Build strong, professional relationships with clients using a trauma-informed and strength-based approach
- Advocate on behalf of clients with external services and stakeholders
- Maintain accurate and timely case notes and documentation
- Participate in case reviews, team meetings, and reflective practice
About you:
To be successful for this role, we’re looking for someone with:
- Experience working with people experiencing homelessness or complex needs
- Qualification in Community Services, Social Work, Human Services, or a related field or similar experience
- Understanding of trauma-informed care, person-centred, and strength- based case management
- Ability to comply with organisational safety procedures, including risk assessments and safety planning for outreach work
- Ability to manage challenging situations and respond appropriately to risk
- Strong communication and interpersonal skills
- Ability to work both independently and as part of a team
- High level of organisation and documentation skills
- A current driver’s licence and willingness to travel for outreach
- First aid certificate and CPR or willingness to obtain
Employment is subject to a satisfactory National Police Check. You will also be required to obtain a NDIS Worker Screening and Working with Children Check
Desirable: (but not essential)
- Knowledge of the local homelessness service system
- Experience using SHIP or similar client management systems
